Community Healthcare

Somerset NHS Foundation Trust runs a huge range of health services, located right across the county and serving a large and varied population. In addition to running the two large District Hospitals in Taunton and Yeovil, the trust runs community hospitals, mental health services, district nursing and even some GP surgeries.

Who We Support

Somerset NHS Charity supports healthcare projects across the county including funding additional equipment for our 13 community hospitals. The hospitals we support are:

Bridgwater Community Hospital, Bridgwater Community Hospital, Burnham on Sea War Memorial Hospital, Burnham-on-Sea Ward, Chard Community Hospital, Crewkerne Community Hospital, Dene Barton Community Hospital, Frome Community Hospital, Marshfield Ward, Minehead Community Hospital, Shepton Mallet Community Hospital, South Petherton Community Hospital, Wellington Community Hospital, West Mendip Community Hospital, Williton Community Hospital, Wincanton Community Hospital.

We also provide funding for mental health services, both for adults and for children. There are mental health sites in the East and the West of the county and demand for these services is growing. We recently funded books about anxiety for our Child and Adolescent Mental Health Services (CAMHS).

Somerset NHS Foundation Trust also runs a large number of GP surgeries and we plan to use donations to make improvements at these surgeries.

The county’s District Nurses are an additional area of focus for us and donations we receive will be used to ensure that the District Nursing teams have everything they need.

A Day in the Life of a District Nurse

We are proud to support the district nursing service across Somerset NHS Foundation Trust.

Our District Nursing service provides high quality nursing care to individuals in their own homes and residential homes across Somerset. The service works in partnership with patients, their family and carers, primary care, acute care, social care, independent and voluntary sector, specialist community health teams, older persons mental health teams and independent rehabilitation teams.

Our district nursing teams are each led by a team leader and include community registered nurses, assistant practitioners, registered nursing associates and healthcare assistants.

Amy, our community and events fundraiser had the pleasure of meeting Cara and Emily from the Taunton team  to find out more about what a typical day looks like for a District nurse and how charitable funds can make a difference.
